All passport/ID card applications of children under the age of 18 have to be submitted by both parents (with parental responsibility) in person as a general rule. If only one of the parents is able to be present, the other should sign a consent letter.
The consent letter of the absent parent must be either a private document with full probative value (signed in front of two witnesses) OR a notarised deed (notarised at a Notary Public, then authenticated by the Consulate).
The form must be filled in and signed with a blue ink pen, in block capitals. As of July 1st 2020, in case of a passport application, the statement of consent can only be accepted within 15 days of its signing. Please note that it is also necessary to attach a good quality copy of the absent parent’s passport and the witness' passports.
